INTRODUCTION The controversy concerning whether or not extraction of bicuspids for orthodontic purposes causes temporomandibular joint disorders (TMD) has been argued for over 20 years. It began in the late 1980s after litigation on behalf of TMD patients claiming that the cause of their problems was orthodontic extractions (McNamarra, 1997). The question is still being debated, although there seems little evidence that removal of teeth actually causes TMD. Computed Tomography in Dentistry History In conventional radiology, anatomy is depicted in only two dimensions. The depth or thickness of structures cannot be measured on projection images. For a comprehensive evaluation of morphology of a structure, data related to the third dimension are essential. For nearly three quarters of a century, multiple, different angle views were used in routine radiography to gain three-dimensional perception of structures through triangulation. In a continuing quest for methods to access data in the third dimension, techniques such as stereoscopic pair image acquisition and tomography were developed. As Neely notes, useful screening methods are still a matter of considerable debate (4:367). Distinctive populations also present the disease at higher rates (e.g., geographically distinct groups or periodontitis associated with systemic disease). There are also considerable differences in the definition of the disease and the term juvenile periodontitis sometimes refers simply to the localized form of the disease (2).

Excessive plaque formation, however, may also contribute to diseases such as gingivitis and peridontitis. Therefore, bacterial proliferation in the mouth requires some form of control. Various means can be employed to maintain microbial populations at levels compatible with health. Currently, antimicrobial agents are used. Perhaps in the future additional methods will evolve.
